NT-proBNP — N-terminal pro-B-type Natriuretic Peptide

1. What is NT-proBNP?

NT-proBNP is the N-terminal fragment of the prohormone B-type natriuretic peptide, released by cardiac myocytes in response to myocardial stretch and wall stress. It is the gold-standard cardiac biomarker for detecting and staging heart disease (mitral valve disease, cardiomyopathy) in dogs and cats.

7. FAQ

Q1. What does an elevated NT-proBNP mean?

An elevated NT-proBNP indicates myocardial stretch or wall stress, most commonly from heart disease. In a dyspneic patient it shifts the differential toward a cardiac cause; in a screening context it can detect occult heart disease before clinical signs appear.

Q2. Why do dogs and cats have different NT-proBNP cutoffs?

Cardiac size, physiology, and normal circulating peptide levels differ between species. The canine reference is below 900 pmol/L, while the feline reference is below 100 pmol/L — the two are not interchangeable.

Q3. Can NT-proBNP diagnose a specific heart disease?

No. NT-proBNP indicates cardiac involvement or stretch, not the specific disease. Echocardiography is required to identify the underlying disease (e.g., mitral valve disease, cardiomyopathy).

Q4. Does kidney disease affect NT-proBNP?

Yes. Reduced glomerular filtration can elevate NT-proBNP, so the result should be interpreted in the context of renal function.
